Change of Membership Form

Thanks for being part of our HAC community! This form will allow you to request changes to your membership account. (E.g., upgrade from an adult membership to a family membership; change your billing status from annual billing to monthly billing, etc.) These changes must be requested by the 20th of the month in order to take effect that month. If this form is received on the 21st of the month or later, changes will not take effect until the following month. We enjoy having you here and appreciate your support of HAC!
